Soul Transposition in Dark Souls 3 is the process of converting a Boss Soul into a Weapon, Ring or Spell. In order to create items you must have the specific Boss Soul, you must have the Transposing Kiln which drops from Curse-Rotted Greatwood, and you must speak with Ludleth of Courland in Firelink Shrine. Below is a list of items that can be created through Soul Transposition, what Boss Souls are required for them and how much it costs.

Note that through Ludleth, Twin Princes' Greatsword can be created by fusing Lothric's Holy Sword and Lorian's Greatsword.
Boss Souls and other special souls in Dark Souls 3 are powerful items that can be converted into new Weapons, Magic, or Items via Soul Transposition from Ludleth of Courland. They may also be consumed to gain a large amount of souls.

Warning,
you have to give ludlet the transposing kiln from curse rotted great wood before putting all the lord of cinders in the trones and burning them, otherwise you can't transpose souls of bosses.

IF THIS IS YOUR 1ST PLAYTHROUGH: Do not be tempted to quickly transpose souls for weapons if they can also be transposed for sorceries/pyromancies/miracles. Even if you're doing a pure melee build, at the end of the game you might decide to get all the achievements, and you need all the magic spells for that. Save yourself from grinding through optional bosses in NG++. Note that you still need to complete the game 3 times to get all ending achievements (but you can start a new character for those if you're careful with transposition, letting you play different builds without respeccing).

you left out the item that is is made by combining Lothric's holy sword and Lorian's Greatsword. Forget what its called but it has the art (Stance+R2) of both.

Making Soul of the Nameless King has 3 instead of 2 like the rest was their way of forcing players through the game a 3rd time.

Theres a soul that you can get after Rosaria is found dead (the covenant leader) and Ringfinger Leonhard has taken her soul to his world where you must hunt him down for it much like the Firekeeper of Dark Souls. It can be traded with Ludleth for a miracle that has great HP regen and affects allies in a AOE.

If you have both Lorian's and Lothric's transposition weapons, Ludleth will ask if you want to transpose the two blades together to create Twin Princes' Greatsword, which does physical/fire damage and has a special Light magic thrust and Fire sweeping attack, similar to each boss.

If you got both weapons (Lothric's Holy Sword and the Lothric's Greatsword) you can craft them both together to the Twin Princes' Greatsword with the help of Soul Transposition in an extra dialouge with Ludleth of Courland. Having both Twin Prince's Swords will allow you to transpose it into one sword called Twin Princes' Greatsword. I'm not sure if that is the only weapon that allows you to combine into one (for instance, both the spell and swords you get from the Dancer don't seem to do anything together) so maybe that is the only items that do?

You can get the Bountiful Sunlight Miracle if you transpose Soul of Rosaria. Seems like the better option since she gives you nothing if you revive her (Unless you absolutely want to respec or change your appearance on that run through)

honestly, they shouldve made both pontiff weapons fuseable too.
